Informed Consent Agreement

My submittal of this contact form to Anisha Zo- Soul Nexus, and Numinosity Institute, LLC, is my signature of agreement to this informed consent and my conveyance that I have read this informed consent carefully and agree to its contents.

I understand that Anisha Zo’s work is to help me connect to my own internal wisdom and guidance, for soulful and spiritual awareness, education and inspiration. I understand Anisha’s work is intended to be a complement to appropriate licensed medical and psychological care, never a substitute for. I understand that Anisha is not a licensed medical, psychological, or psychiatric professional and that she does not diagnose or treat.

I understand that Anisha sees her work with me as sacred. She honors my privacy and holds in confidence the contents of our session.

I agree to give Anisha notice of cancellation or need to reschedule an appointment as close to twenty-four hours in advance as possible.

I understand the purpose of Anisha’s work is to help me connect to my own inner wisdom, intuition, and divine Spiritual guidance and support.

I understand that Anisha works with subtle energy (psychic and spiritual) and that this energy is often expressed and communicated in symbols and metaphors. I agree to take responsibility for interpreting the contents of my session and any action or non-action I take relative to the contents of my communication with Anisha.

I understand that in working with subtle energy, that accuracy is not guaranteed, nor can Anisha choose what spirit comes through in my session. Often what comes through takes time to process. I agree to be open-minded to the natural subtle energy process of my session.

I understand that if I have been diagnosed with a serious mental health diagnosis that before scheduling a session, I inform my licensed health care team of my seeking a spiritual connection session (psychic/mediumship) and get their okay that a session is in alignment with my well-being and agree to follow the guidance of my healthcare team as to whether or not to go forward with a session.

I understand that if I am challenged by addiction, that drugs and alcohol hold energy that can interfere with my session. I will come to my session at least 48 hours sober.

I agree not to hold Anisha Zo Kuhn, John Kuhn, or Numinosity Institute LLC, legally responsible for my experiences working with Anisha Zo-Soul Nexus.
